Work Package 3 - National level experience
Status: Completed.
This work package aims to understand how PSIRF is operationalised and implemented over time in two ways:
- A national survey of all NHS organisations implementing PSIRF In England at two time points (during the early implementation phase and after a period of embedding of the policy). The survey will collect information about key PSIRF implementation activities and will measure implementation processes using the Normalisation Process Theory NoMAD instrument. The first survey was open between October and December 2023. The second survey was open between May and July 2025.

- As part of PSIRF implementation, NHS organisations are required to develop a Patient Safety Incident Response Plan (PSIRP). We are undertaking a documentary analysis of PSIRPs (reflecting variation in region, organisation size, type of healthcare provider etc) to explore how PSIRF is interpreted and enacted across organisations.
